Frequently asked questions.

Q: Who can complete a DVLA Driver Medical?
A: Any doctor who holds full registration with the GMC (General Medical Council) and is licensed to practise medicine in the UK. On Road Medicals has a panel of 60+ doctors annually trained in Group 2 medical assessments.

Q: Where can I get a driver Medical form from?
A: All our doctors carry copies of the D4 medical form, however we always advise bringing your own copy to your appointment. You can download the D4 medical form off our website. For Taxi Medical forms, some clinics may carry council forms, though not reliably—so best to check ahead.

Q: Will the doctor help me complete the D2 or D47P form?
A: No—the doctor completes the D4 medical form for you, but you will need to complete the D2 form yourself. For more information on how to complete your D2 or D47P form please visit the DVLA website.

Q: What happens during a D4 medical assessment?
A: During the assessment:

  • An eye test (with glasses/contact lenses if applicable)

  • Blood pressure check

  • Listing of any medication currently taken

  • Questions about your medical history (these are compulsory)

  • Possibly a urine sample to test for diabetes
    At the end: you sign the back page of the form to confirm correctness, check the form is fully completed, and send it to DVLA. DVLA reviews and either accepts or rejects, explaining any reasons if rejected.

Q: How long does a medical take?
A: Approximately 15 minutes. If your medical history is complicated, it may take longer.

Q: Does the D4 Medical include an eye test?
A: Yes. All medicals include an eye test at no extra charge. You will be asked to complete it with and without glasses/contact lenses. Bring your lenses container if needed.

Q: What do I need to bring to my appointment?
A: You should bring:

  1. Driving licence or Passport

  2. Bank statement or utility bill for proof of address

  3. A list of any medications you take (if applicable)

  4. Glasses or contact lenses (if worn)

  5. D4 form

  6. Outstanding payment in CASH

Q: How long is the D4 medical valid for?
A: The DVLA D4 medical form is valid for 4 months from the signature date. The paperwork must be sent to DVLA within those 4 months—especially important for group 2 licence categories.

Cancellations / Rescheduling

Q: What is your cancellation policy?
A: You must provide at least 24hrs’ notice (Monday to Friday) to cancel a medical appointment. If given, the fee is refunded. If cancelled within 24hrs, a cancellation fee applies.

Q: Can I rebook?
A: Yes. Rebooking is possible for a reduced fee. You need to call our phone lines; if booked online again, the full medical price is charged. Common rebooking reasons include missing forms or incorrect documentation.

Q: Can I still show up without my forms?
A: Not ideally. The medical summary forms are necessary. If you don’t have them in time, let us know at least 24 hours before the appointment and we may reschedule without charge. Turning up without your summary may lead to the appointment being cancelled or charged.

Taxi Medical Questions

Q: Do I need to bring my medical records to my taxi medical appointment?
A: Yes—many councils require either NHS App access or full medical record from your GP. Without them, the doctor may not be able to complete your form.

Q: How can I get my medical records?
A: You can request them via:

  • The NHS App

  • Asking your GP receptionist

  • Using/emailing our free request template (on the Request Records page)

Q: Where can I get a Taxi Medical form from?
A: Some clinics may carry council forms, but not always. Your council may send them, or they can often be found online.

Q: Does the Taxi Medical include an eye test?
A: Yes. Same as the D4 medical, includes eye test at no extra charge. Bring glasses/contact lenses if applicable.

Q: What do I need to bring to my appointment (Taxi)?
A: Similar to the HGV / D4 medical: licence or passport, proof of address, list of meds, glasses/contact lenses, any required forms (D4 or council) and medical summary, plus cash payment.

Q: What does a Taxi Medical include?
A: Completed by GMC‑registered doctors to a Group 2 standard. It includes: medical history review, physical examination (vital signs, etc.), vision & peripheral vision tests, possibly urine test (if required by council), ID and proof of address checks.

Q: Can I have a receipt for my accountant or employer?
A: Yes. You will receive a VAT receipt via email alongside your appointment confirmation.
